APPLY

Requests can be made on behalf of individual people who have identified needs and inadequate resources to meet them. Each application must be made in writing in advance by one of the below mentioned professionals.

APPLICATIONS CAN ONLY BE MADE BY:

  • Health Professionals.
  • Accredited Social Workers.
  • Resource Teachers.
  • Counsellors.
  • Or other approved community workers who are familiar with the individual's case history.

Please note that applicaitons will not be accepted from November the 30th through to March the 1st.

AN APPLICATION SHOULD INCLUDE THE FOLLOWING:

  • Full information of the person who will benefit (name, date of birth, address);
  • Purpose for which monies are requried and details of the circumstances and family background;
  • Amount that is required;
  • Identity of the provider of the goods and service.

An application form can be completed online and emailed to info@hymanmarkstrust.co.nz or printed and posted to PO BOX 35-135 Shirley CHCH.

WHAT HAPPENS NEXT?

The Trustees meet fortnightly. If approved the grant will be paid directly to the service provider who will submit their invoice directly to the secretary's office for payment.

When deciding to agree to a grant the trustees take into account:

  • The objective, is it realistic and within the Trusts terms of reference.
  • Is there obvious hardship.
  • Will it help someone who is working hard to help themselves.
  • Have all avenues of assistance been explored.
  • Will it have a lasting impact.
  • Would Hyman Marks approve of the distribution.
